ADULT MODEL RELEASE

       I, ____________________________________ ("Model") for good and valuable consideration, the receipt of which is acknowledged, give to Heidi Hardin ("Artist"), her legal representatives, successors and all persons or corporations acting with her permission, the irrevocable right and unrestricted permission to use, copyright and publish, my name (or any fictional name), picture, or photograph, in all forms and media, in all manners, including but not limited to, negatives, transparencies, prints, or digital information pertaining to them, in single, multiple, moving or video format, in which I may be included, in whole or in part, or in composite or distorted form, or reproductions thereof, in color or otherwise, for all media, art, trade, advertising or any other lawful purpose in connection with the Artist’s project entitled: The Human Family Tree/A Walk Through Paradise….

I herby waive any right to inspect and approve the finished product, version or copies thereof, or the use to which the Artist has applied my image, including written copy that may be created in connection therewith.

I agree that all ownership, title, copyright, trademark and interests in any finished product to which the Artist has applied my image, is exclusively the Artist’s, including, but not limited to, display, exhibition and transmission rights.

All disputes arising out of this Release shall be submitted to mediation in accordance with all the rules of the Arts Arbitration and Mediation Services, a program of California Lawyers for the Arts. If mediation is not successful in resolving the entire dispute, any outstanding issues shall be submitted to final and binding arbitration in accordance with the rules of that program. If such services are not available, the dispute shall be submitted to arbitration in accordance with the laws of the State of California. The arbitrator’s award may include the costs and attorneys fees of the parties to the arbitration. The award shall be final, and judgment may be entered upon it by any court having jurisdiction thereof.

I am not a minor. I have read this release and agreement prior to its execution and I am familiar with its contents.
